The reassignment of identity in traditional societies isn't all that unusual. In ancient Egypt it at least included the visual representation of Haptesuit (sic) although we will probable never know how this translated into her everyday life.

A related issue in Muslim society is the view about changing identity. For example the Iranians loathe homosexuality but accept it if the man will undergo sexual reassignment surgery. Their rates of such surgery are amongst the highest in the world. Therefore for them the sexual behavior issue is about sexual identity and if you do the surgery its all OK. For them men who want to have sex with other men are really women and this can be fixed with the surgeon's knife. For such men its a better option than a public hanging. To the Iranian leadership identity is not about the mind but just the plumbing.

There is a related issue of unsustainably rigid systems and breaches.

The Afgans have rigid systems on identity and sexuality - which they break regularly - as is often the case in traditional societies. For example there are multiple media reports on the role that young boys play in this sexually rigid society. In particular they do dancing for the elders and tribal leaders - probably followed by sexual contact. In societies which rigidly segregate men and women these things are not all that unusual - although they violate the religious standards they claim to uphold.

And yet some seemed to accept the position quite happily, not that that justifies what they go through. I watched this quite a long time ago, there are others on You Tube if you wish to get a better understanding/insight into these events.

You you see why many people think the West is mad to pour billions of dollars into a country so utterly corrupt that they cannot/will not deal with this well-known practice even though it's contrary to the teachings of Islam

But so is the trafficking of heroin .....90% of the world's supply grows in Afghanistan.
